Aged
Health & Rehabilitation Program
Services are provided for frail aged and disabled persons, their
cares and families within the cities of Port Phillip and
Stonnington; adults requiring community based physical and social
rehabilitation within the City of Port Phillip; and adults
discharged from acute hospitals, subacute services and those
presenting to emergency departments within the inner south of
Melbourne.
Community, Youth and
Family Health Program
The program provides and extensive range of multidisciplinary
centre-based and outreach services to children, young people and
families.
Mental
Health Program
The program provides a wide range of community supports and
services to people aged 16 to 64 years who have and enduring
psychiatric disability. The program enables clients to maximize
their options and live satisfying independent lives.

Dental
Health Program
Located at the Prahran and South Melbourne sites the program
provides dental to Health Care card holders. A fee is charged.
However, the Dentists for Homeless People, also known as Outreach
Dentistry provides a free service to homeless people including
people in rooming houses, Special Residential Services and places
where vulnerable clients might be accessed.
